Outreach that sustains independence.
Rathbone began providing outreach support in Lambeth in 1990. We now support over 80 people, or ‘service users’. The main aim of the service is to maintain the independence of individual people with learning disabilities so that they can remain living in the community and not need to move into in a care home or supported living.
The people using the service have a wide variety of needs. Some may only need a couple of visits each week to ‘check-up’ on their welfare and offer advice and support. Others may need daily visits to maintain their health and wellbeing. Some people we work with may have elderly carers and we offer support to ensure the person can access their local community and take part in meaningful activities.
