Is this service right for the person you support?

Who it's for
Adults with learning disabilities who live in their own home, with family or in other community settings and need some support to stay safe and independent. This may include people with autism, mental health needs or physical health needs. Support can range from short “check-in” visits each week to more intensive daily support, depending on what the person needs.
Age range
Adults aged 18 and over with a learning disability.
Cost & funding
Outreach is usually funded by the local authority as part of a person’s social care package, based on the number of hours of support they need each week. People may make a financial contribution depending on their benefits and savings, following the council’s charging policy. We can explain how charging works and what it means in practice before support begins.
When support is offered
Support is mainly provided during daytime and early evening, Monday to Friday, with some flexibility at weekends where care plans require it. Visit times and frequency are agreed in advance and can be reviewed if someone’s needs change.

How to refer

Refer someone to Outreach Service

Most people are referred by a social worker, care co-ordinator or health professional after a social care assessment. Families and carers can ask Lambeth Adult Social Care for an assessment if they think outreach support might help. Professionals from other boroughs can also refer if they are funding the package. We are happy to talk through potential referrals informally before paperwork is completed.
